Our Mission

To provide integrated housing, employment, health, and advocacy services for individuals transitioning from treatment or incarceration — bridging the critical gap between discharge and self-sufficiency, and breaking the cycle of addiction, incarceration, and homelessness.

Our Vision

A community where no person loses their recovery or their future for lack of housing, support, or a fair chance — where every individual leaving treatment or incarceration has the runway and resources they need to build a stable, dignified, and self-sufficient life.

Guiding Principles

Our Core Values

Dignity

Every person who walks through our doors is treated with unconditional respect, regardless of their history, diagnosis, or circumstances. We see the person, not the past.

Integration

Housing, health, employment, and advocacy are not separate problems — they're one problem viewed from different angles. Our services reflect that reality.

Persistence

Recovery and reentry are not linear. We don't give up when someone stumbles. We walk with them for as long as it takes, adjusting the plan, not the commitment.

Equity

We intentionally serve those whom the system has left behind — the people who fall between programs, who are "not quite" eligible, who need a fair chance.

Transparency

We hold ourselves accountable to our participants, our funders, and our community through measurable outcomes, honest reporting, and open governance.

Community

We don't work in isolation. We build partnerships with providers, employers, faith communities, and government agencies to create a seamless safety net.

Why We Exist

The Gap in the System

PRC Integrated Care Services was founded to address a failure in the social services system that affects thousands of people every year: the gap between discharge and self-sufficiency.

When someone completes substance abuse treatment, they may leave with a discharge plan and a commitment to sobriety — but no housing, no income, and no runway. When someone is released from prison, they may have parole conditions and good intentions — but no place to sleep and no job waiting. In both cases, the system hands them a piece of paper and sends them into a world that isn't ready for them.

The result is predictable: relapse, recidivism, and homelessness. The person who was clean yesterday uses again because they're sleeping in their car. The person who wanted a fresh start reoffends because they can't survive on the street. The cycle continues, and the community pays for it in emergency room visits, jail beds, and lost human potential.

PRC Integrated Care Services is the bridge. We provide the housing, the case management, the financial assistance, and the advocacy that fills the gap — for those crucial 30 to 120 days when someone is most vulnerable. We don't replace treatment or the justice system; we complete them, by ensuring that what they started doesn't end on the street.

Our integrated model recognizes that housing alone isn't enough, and employment alone isn't enough. People need all of it — at once, coordinated, delivered with respect — to make the transition from dependency to dignity. That's what we do.
